Administered by the Keene Housing Authority, the Congregate Housing
Services Program is available at two public housing sites.
The CHSP provides a supportive residential environment that allows
individuals, especially frail elders, to live independently, therefore
avoiding premature or unnecessary nursing home placement. CHSP also
supports deinstitutionalization, offering opportunities for elders
and disabled adults to return to the community after an assisted
living stay. Residents live in their own apartments and receive
supportive services.
Good nutrition and socialization are important; the CHSP provides
a nutritious midday meal served daily in a social setting. CHSP
participants receive assistance obtaining transportation to and
from medical appointments. Basic homemaking services and limited
personal care assistance are arranged based on the individuals assessed
need. CHSP also offers educational workshops and special social
events throughout the year. Some events are made possible with the
help of area college students and other community volunteers. A
service coordinator monitors the program and provides case management.
